Enrollment Process:

The following must be submitted:

1.
Forms:

  • A completed application form per child
  • A completed emergency form per child
  • A completed tuition payment information form - 1 per family
  • A completed and signed SMART TUITION form

2.
Fees:

  • Registration fee: $250 per student (non-refundable)
  • Instructional fee: $250 per student (non-refundable)
  • Graduation fee:  $100 (eighth grade)
  • ITBS Test fee:    $45.00 (second grade - eighth grade)
  • SMART TUITION - $35 (non-refundable) annual fee will be deducted from your account at the time of
    enrollment

NEW STUDENTS & FAMILIES, in addition to the above, please submit the following:

  • Birth certificate (must accompany registration)
  • Baptismal certificate (if Catholic)
  • Florida physical exam (yellow card)
  • Immunization in compliance with Health Department Regulations (blue card)

A child will not have a place reserved until the applicable forms, payments, and paperwork (above) are
received by the school. No student will be allowed to attend the first day of school without the appropriate
paperwork.
